肩を回した時の音と痛みが主訴の方が来られました。両肩に同じ症状があるのですが、右肩により強く症状が現れています。
発症時期は15年くらい前。
痛みは肩の上側にあるとのことだったが、はっきりしない様子でした。
こういう場合は肩後部に問題があることが多いので、触診してみるとかなり強い圧痛がありました。
ここが主訴の原因であれば、この患者さんの場合、肩関節後部のインピンジメント症候群ということになります。
これは、肩をある一定の方向へ動かした時、上腕骨（頭）と肩甲骨（関節窩）の後部が衝突（インピンジメント）している状態です。
ただし、骨同士（この場合、上腕骨頭と肩甲骨の関節窩後縁）がぶつかるのではなく、その間に軟部組織である棘下筋腱や関節包が挟み込まれます。
通常、これらの軟部組織が痛みの原因構造となっています。
この患者さんの場合、肩の動作に伴い音があるので、おそらく棘下筋腱が関節周辺の骨の突起部分とこすれている音だと思われます。
以上の考察を元に治療を開始。
治療後は音も痛みもゼロになりましたが、15年来の症状が1回の治療で寛解したなどとは、全く思っていません。
とりあえず、セルフケアの方法をお伝えして、その場は終了となりました。

A patient came to our office with a chief complaint of noise and pain when rotating the shoulder. He has the same symptoms in both shoulders, but the symptoms are more intense in the right shoulder.
The onset was about 15 years ago.
He said that the pain was on the upper side of the shoulder, but he was not sure.
In such cases, there is often a problem in the posterior part of the shoulder, so I palpated the shoulder and found that there was quite strong tenderness.
If this is the cause of his chief complaint, in this patient’s case, it would be posterior shoulder impingement syndrome.
This is a condition in which the humerus (head) and the back of the scapula (glenoid fossa) collide (impingement) when the shoulder is moved in a certain direction.
However, the bones do not collide with each other (in this case, the humeral head and the posterior border of the glenoid fossa of the scapula), but rather the soft tissues, the infraspinatus tendon and the glenoid capsule, are interposed between them.
Usually, these soft tissues are the pain-causing structures.
In this patient’s case, there is a sound with shoulder motion, which is probably the sound of the infraspinatus tendon rubbing against the bony projections around the joint.
Based on the above considerations, treatment was initiated.
After the treatment, both the sound and the pain have decreased to zero, but I do not believe at all that the patient’s symptoms of 15 years have gone into remission after one treatment.
At any rate, I ended the session by telling him how to take care of herself.
